Report a workplace injury

Reporting promptly is the best thing you can do for your injured employee and for your experience rating. Here’s how to file a claim with UBIC.

After an injury

Get medical care

Arrange appropriate medical treatment, and emergency care if it’s needed. For a life-threatening injury, call 911 first.

Drug test

Arrange post-accident drug testing for every injured worker, per your policy and state requirements.

Report the injury

Complete the Employer’s First Report of Injury and submit it to UBIC / URS promptly.

Offer modified duty

Where you can, provide modified or light-duty work within the employee’s restrictions to support recovery.

How to report a claim

Medical emergency?

If the injury is serious or life-threatening, call 911 first, then report the claim once the employee is safe.
